How does the insurance work..does the surrage's insurance pay for the dr visits and delivery, or does the insurance not cover any expense

What coverage a surrogate mother may have with her own health insurance will vary by situation. Most, though not all, health insurance policies that a surrogate mother has obtained through her employer or her husbands will not specifically restrict her from being covered as a surrogate.
What this means is that in many cases, the health insurance she already has will cover the maternity portions of the journey: namely the doctors visits and the birth.
A surrogates health insurance policy will not in any way cover the IVF clinic fees or fertility treatments. Those are the responsibility of the intended parents.
Some surrogates have had health insurance policies that specifically restrict any form of surrogacy. If your policy restricts this, it will be in your benefits/ policy book.
All surrogate mothers will need to forward this book to her attorney, so that (s)he can determine if the surrogacy is indeed allowed.

Its worth it to send in the surrogate mother's health insurance book to the attorney. MAKE SURE her insurance covers the surrogate pregnancy BEFORE you start using it to prevent problems later on.
